NURS FPX 4010 Assessment 4: Stakeholder Presentation

Introduction

Stakeholders form a key part of the project, therefore, they need to be involved in the project from the onset. In addition, it is important to effectively, constantly and efficiently communicate with them to obtain buy-in and ensuring that they appropriately support the proposed solution or project. Therefore, as part of the project plan, the project team should present a plan to the stakeholders to help generate their interest and buy-in for the plan proposal (Hopp & Rittenmeyer, 2020). Therefore, the purpose of this presentation is to obtain buy-in and interest among the stakeholders and leadership groups regarding the proposal which had been developed

The Patient Issue

A comprehensive interview was done which helped reveal various patient care problems which affect the facility. The identified patient issue is increased readmission rates in the hospital, especially in the psychiatric units. This is a major issue as it has been connected to a host of negative impacts on patients, healthcare facilities, and patient families. Readmissions mean that patients become more exposed to hospital-acquired infections, which further complicates their lives (Owusu et al.,2022). In addition, the patients and their family members may need to spend more upon readmission into the psychiatric units. It is important to note that various nursing strategies or nursing interventions can be used to help fight the problem.

Hospital readmissions among patients with mental health challenges result from various factors such as relapses, failure to adhere to the treatment and management plans, and failure to follow the recommended or prescribed lifestyle. This issue can be solved using an interdisciplinary approach since the problem may emanate from failures in different parts or points of care, from patient admission up to the time when the patient gets discharged(Owusu et al.,2022). The implication is that an interprofessional collaboration between the healthcare professionals working in these points of care can be important in reducing the readmission rates in psychiatric units. Therefore, as part of the proposal plan, various strategies that can be used to fight this issue will be explored as appropriate.

Summary of EBP Interdisciplinary Plan

Interdisciplinary team approach has been shown to be important in solving various problems facing patients in the care environment. Therefore, it is important for professionals to consider solutions that focus on interdisciplinary approaches (Olotu et al.,2019).  One of the reasons why interdisciplinary approach is relevant or important to this issue is that it enables the provision of a holistic patient care since every member is able to use their expertise to help solve the issue. It also enables the use of specialized expertise who can help in better management of patients. Interdisciplinary approach also enhances care coordination which then ensures that patients get appropriate follow-up care to reduce chances of readmission. It also encourages the use of patient-centered care as patients can be put under individualized care plans to improve their conditions and minimize the chances of readmissions. This approach can also help in medication management since hospital readmissions have also been connected to non-adherence and medication errors.

As earlier indicated, it is important to use an interdisciplinary approach is solving the issue of higher rates of hospital readmissions. The use of readmissions can help in the achievement of improved outcomes or the project goals in various ways. For example, it ensures that the patients can get the needed follow-up and support upon discharge. Such support and follow-up will ensure that the patients adhere to the prescribed medications as well as lifestyle modifications to help prevent the possibility of readmissions hence achieving the goals. Interdisciplinary approach also ensures that carry out a regular review of the readmission cases and identify potential opportunities to improve. The implication is that the continuous quality improvement approach can help the organization to refine the approaches for lowering the readmission rates as time goes on (Dang et al.,2021). The members of the interdisciplinary team can also help carry out a comprehensive assessment of patients before discharge and obtain useful information regarding the patient social support, home environment, mental health, physical health and various barriers to the follow-up care. Such an approach can lead to the development of specific plans which can then help reduce the rates of readmissions.

Interdisciplinary Plan Summary

The interdisciplinary plan helps as it acts as guide for implementing the interdisciplinary solution. The major aim of the proposed interdisciplinary solution entails timeously identifying patients who can be prone to readmissions and using various strategies to reduce that risk. Some of the strategies may include teaching the patients and their family members regarding the importance of adhering to the medication regimen, following the prescribed lifestyle modifications, and ensuring that the patients get the necessary support during discharge to help them better cope with symptoms upon discharge (Cui et al.,2019). Therefore, the aim of the approach is to lower the rates of readmission by at least fifty percent. The are higher chances of the solution working since each member of the interdisciplinary team will be tasked with bringing their expertise on board to help reduce the rates of readmission. Each member of interdisciplinary team will play a critical role. For example, nurses will be tasked with patient education, pharmacist will be tasked with medication review while physicians will be tasked with appropriate prescription.

Implementation and Resource Management

The implementation of the interdisciplinary solution will follow a defined strategy. It is also important to ensure that the implementation of the project follows the plan to ensure that the resources are effectively used. The implementation will follow the phases of a normal EBP implementation plan which entails several phases. The phases include formulation of the project objectives, searching for the evidence and appraisal, engagement of interdisciplinary team, developing the guidelines, piloting, educating the healthcare professionals and implementation (Dang et al.,2021). Following the guidelines or stages will enhance appropriate use of the resources to avoid any waste. The plan also justifies the use of resource expenditure since the plan outlines various resources needed in each phase. Such resources will determine the nature of expenditure.

EBP Criteria For Evaluation

Evaluation is an important part of an EBP project. Therefore, it is important to come up with an evaluation criteria. The project would be regarded successful if the rates of hospital readmissions are reduced by at least fifty percent. It is expected that the rates of readmissions will reduce upon the implementation of the proposed solution. The rates of readmissions will be obtained from the hospital sources. Such rates observed after the implementation of the project will be compared to the rates observed before implementation to determine the effectiveness of the implementation solution (Dang et al.,2021). Therefore, it will be easier to determine the degree of success since there will be a reflection of how the rates of readmissions have been reduced within the study period.
